Either way, with a traumatic injury like yours she would most likely send you for x-rays before doing...

In my limited experience surfing north and south baja, 4x4 isn't really necessary unless you make a bad decision. If there is any chance of rain though, DO NOT GO! More important than 4x4 would be...
