What's really causing boating accidents like this?

Is it really weather that is causing accidents like the ones in the article above? It seems like it's so easy to check the weather before you leave (and even during your boating outing). I even check weather through my Navionics app (Menu ->Weather and Tides). Is it something boaters are not doing (or are doing)? What is really causing things like this? How can we be better at preventing needless accidents like this?

Some people are idiots.
Some never check the weather.
Some ignore the signs of weather coming.
And some have the misfortune of having a storm come out of nowhere. And it is amazing how big waves can get really fast. Then you need both luck and skill.
